Beacon After School Registration

REGISTRATION PROCESS:
 
Please see below the documents that will be required to enroll your child in afterschool:
  1.  
  2. DYCD Application on DiscoverDYCD [https://discoverdycd.dycdconnect.nyc/home] Search for the program using Code: BEA-13426
     
  3. **Medicals are required and are valid for 1 year from the date the physical was completed. Your child will not be allowed to start the after-school program without it.
     
  4. Current picture of your child(ren) and additional documentation (Emergency Contact card and signed consent forms)
     
  5. Parent Orientation VIA Zoom on Thursday, September 5th at 5 PM. 
    (The Zoom link will be sent out at a later date)
**Please drop off the Medical form and current picture of your child to the Beacon Program at IS5. Additional documents will be provided to you to complete on-site. 
**Document Drop-off dates are Tuesday, September 3rd, from 3 PM to 6 PM and Wednesday, September 4th, from 10 AM to 12 PM.
**All Documents must be submitted to the Beacon Program at IS5 to complete enrollment and secure a spot in the program
**Any family unable to get documents in on time or, if there are no more seats in the program, will be placed on a waitlist and called back once a seat becomes available; documents will be due at that time.

*Please note the Beacon Team will be on vacation starting Friday evening, August 16th, and will be returning Wednesday, August 28th. Any emails sent between that time will receive a response upon our return to the best of their abilities.*
