Good Day, I.S. 5 Students and Families! 👓
On Monday, students received a form for families to complete if you wish to OPT OUT of the event. If you do NOT want your child to have an eye exam or, where applicable, a FREE pair of glasses, you must submit a completed OPT OUT form and give it to the homeroom teacher. If we do not have your OPT OUT form, students will be participating in Optical Academy services! 👓
The word has been out since December to parents in all of our meetings, the schedule has been sent out to staff, OPT OUT forms distributed to students and now we need to make sure everyone is aware of the exciting days ahead with Optical Academy! This is a reminder about this wonderful opportunity - and we look forward to “seeing” you at the Optical Academy I.S. 5 Event! 👓
